      <description>A Qualified Social Worker is required to join a Hospital Team based in South East London.

The appointed Qualified Social Worker will work as part of a multidisciplinary team, to be responsible for ensuring the provision of individual care packages for adults, including those with complex needs and those where statutory intervention is required. 

The primary objective of the team is to prevent avoidable hospital admissions, reduce the time people stay in hospital, enable people to remain independent in the community and facilitate the provision of long-term care where returning home may no longer be a safe option. The senior care manager will be involved in assessing individual need in conjunction with other relevant professionals, and the design of creative and innovative packages of care. The role also involves negotiating the funding for care plans, monitoring their effectiveness and providing support for clients to ensure their success, with an emphasis on promoting independence. The role will also be educative and consultative.  

The Senior Care Manager will be encouraged to contribute to the development of the service through their participation in research and relevant working groups. To provide a service to those eligible, in acute and non-acute hospital settings as well as those discharged into the community.

      <title>Senior Social Worker London 554658RK</title>
      <description>A Senior Social Worker is required to join a Care Planning team based in North West London on a temporary basis.

The care planning teams develop and implement care plans for children and young people of all ages. Two teams (Care planning 1 and 2) will focus mainly on care proceedings and achieving permanence for children and young people. Court work plays a prominent part in the teams' workload and staff complete in-depth and complex assessments. The teams implement care plans, achieve permanence for children in care and enable them to achieve their full potential. There is close working with colleagues in CAMHS, Placements, the LAC education team and most importantly, with service users and carers to transform plans into good outcomes for our children and young people. Life story work plays an integral part of the work. Section 7 and 37 reports are completed, and the service will also work with families whose children are on Supervision or Interim Supervision Orders.

The work includes:

- All children and young people now in care aged 0-18 
- New admissions to care when it is clear that rehabilitation has been ruled out  - S 20 and S 38
- Remands into local authority accommodation
- PLO care proceedings post the first application, including No Order and Supervision Orders
- S 7 &amp; S 37 reports if a new referral
- Permanency Planning with Placements Service
- Ensure PEPs; Health Assessments and LAC paperwork up to date
- Life story work
- Support, assist and befriend care-leavers age 18 - 21/24
- Work closely with Commissioning and Access to Resources Service
- Unaccompanied minors, who will be referred directly to the Care Planning Service

The appointed social worker will have 2 years post qualified experience of assessment work and will be required to carry out parenting assessments on families. The work will include assessing families in residential and community assessments, writing reports and attending court.

Working within the Children's Referral and Assessment team, this Qualified Social Worker will have the following responsibilities:

- Undertake the assessment of children, families and carers using the relevant assessment framework.
- Develop and participate in relevant care plans. 
- Ensure that service users' views are fully recorded during the assessment process and where possible are reflected in the subsequent case planning.
- Prepare and present reports in a clear and logical way, and contribute to multi-agency conferences and meetings as appropriate.
- To include cost / 'best value' considerations in identification of resources and case planning.
- Undertake single or joint agency enquiries to assess likely and actual harm to children and take appropriate action to ensure their safety.
- Develop and implement child protection plans.
- To lead on complex assessments with children, young people and their families/carers where there are specific concerns requiring a high level of specialist involvement.

The successful Social Worker will be GSCC registered and have a current CRB.

Working within the Child Protection team, this Senior Child Protection Coordinator will have the following responsibilities: 

- To chair child protection conferences and contribute to the development of child protection policies/practice, monitor and evaluate service provision and provide consultation/advice to a wide range of practitioners. 
- To act  as designated officer in respect of allegations  against staff and to support the child death overview process
- Assist the Local Safeguarding Children's Board in the production of the annual report and the development of appropriate polices and procedures on Safeguarding issues.

      <title>Senior Occupational Therapist -Mental Health  South East OT/CROY/OO</title>
      <description>Senior Mental Health Occupational Therapist - South London

We are urgently looking for an experienced Senior Occupational Therapist to work in a private hospital based in South London

The Senior OT will be part of the multi-disciplinary team within the low secure hospital, working with service users who have a severe and enduring mental health problem or learning disability, who may also have challenging behaviour or a forensic history. 

The role will require you to work at least one day a week in Surrey, working with male and female service users.

The main duties of the Senior OT will be to carry out specialist OT assessments with each service user, using the Model of Human Occupation (MOHO) to guide practice, and develop and implement occupational therapy intervention focussing on daily living and vocational skills in consultation/conjunction with the multi-disciplinary team and particularly the Director of Therapeutic Services

Other duties include :

*To manage a clinical caseload of in-patients with severe and enduring mental illness or learning disability who have a forensic history and/or challenging behaviour.

*To undertake and interpret complex and detailed Occupational Therapy assessments, using the Model of Human Occupation (MOHO) to underpin your practice.

*To carry out assessments of activities of daily living to determine a patient's level of functioning
 
*To work as a member of the multi-disciplinary team, carrying out Occupational Therapy intervention with patients, planning individual treatment programmes specific to their needs with particular emphasis on the daily living skills, vocational and leisure occupations of the patients, utilising graded activities to achieve therapeutic goals.

*To take responsibility for the planning, facilitation and evaluation of the group OT programme within the service.

*To provide a consultative role in the planning and delivering of a ward-based activity programme by nursing staff and therapeutic care workers (TCW's).

*To provide focused OT assessment and intervention to service users within Aston Lodge who are referred for occupational therapy.

*To provide OT interventions which are ward-based and community-based, promoting social inclusion and recovery.

*To evaluate OT treatment and ensure outcome measures are recorded

*To take on the role of vocational lead within the hospital

*To utilise skills learned through practice e.g. art, music, gardening

*To communicate patient-related information effectively to ensure collaborative working with multi disciplinary colleagues in reviews, Section 117 meetings, section renewals and Care Programme Approach (CPA) meetings
   


Senior Mental Health Occupational Therapist - South London

The successful Senior Occupational Therapist, must be HPC registered and have at least 2 years post qualifying experience within mental health.

Principal Responsibilities

*Actively Solicit for new business

*Quality Control

Agency Manager - Older Persons Care - East London 

The successful candidate will be managing a team of two senior care coordinators and two administrators and approximately 60 support workers.

The ideal candidate will be hard working, entrepreneurial and work well as part of a team. 

It is essential to have the NVQ Level 4 or above and have at least 2 years experience of working in a management capacity, in a domiciliary care setting

* The policies and procedures of the home 
* Statutory requirements 
* Maintaining set standards of care 
* Maintaining standards of communication with relative, residents and all others 
* Maintaining standards of Health &amp; Safety, risk assessments and home hygiene 
* Any other aspect of activity that may effect the care of a resident 
* To be a named nurse for a group of residents and supervise the key worker 
* Clear instructions are given and received to all staff 
* Record keeping is appropriate 
* Liaise and make appointments with multi disciplinary teams 
* Play an active part in handover, rostering and managing staff support 


Registered General Nurse - Oxfordshire

The successful Nurse's must be Registered and experience of working with Older people is desirable. 
Excellent rates of pay, however this is dependent on experience. 

This is a full-time permanent position with a pay rate of at least &amp;#163;15 per hour and will be an immediate start!
